fa موانع اجرایی مراقبت تیمی و آموزش بین‌حرفه‌ای: دیدگاه‌های مدیران آموزشی نظام سلامت کشور آموزش پزشكي مبتني بر نيازهاي جامعه Community Based Medicine پژوهشی اصیل Original research article مقدمه: امروزه رویکردهای آموزشی بین‌حرفه‌ای، همکاری بین‌حرفه‌ای و مراقبت تیمی به ضرورتی اجتناب‌ناپذیر جهت مقابله با نیازهای سلامتی جوامع تبدیل شده است. تناقض موجود بین ضرورت ارائه مراقبت تیمی در مراکز درمانی و عدم اولویت‌دهی و یا به‌کارگیری رویکردهای آموزشی تیم محور و بین‌حرفه‌ای در برنامه‌های آموزشی نظام سلامت کشور، زمینه‌ی انجام مطالعه‌ای با هدف تبیین موانع اجرایی مراقبت تیمی و آموزش بین‌حرفه‌ای از دیدگاه مدیران مراکز آموزشی نظام سلامت در ایران گردید. روش‌ها: مطالعه‌ی کیفی از نوع تحلیل محتوی و با رویکرد قراردادی در طی شهریور ماه 92 تا تیرماه 93 با مصاحبه‌ی انفرادی نیمه ساختارمند با 16 نفر از مدیران، معاونین و محققان دانشگاه‌های علوم پزشکی و مراکز مطالعات و توسعه آموزش پزشکی کشور، با نمونه‌گیری مبتنی بر هدف انجام شد. کلیه مصاحبه‌ها پس از پیاده سازی با استفاده از نرم افزار MAXQD مورد تجزیه وتحلیل قرار گرفت. نتایج: موانع تبیین شده از دیدگاه مدیران شامل دو طبقه‌ی اصلی و هشت زیر طبقه بود، طبقات اصلی موانع فرهنگی– نگرشی و موانع ساختاری – عملکردی بود، موانع فرهنگی– نگرشی، زیر طبقات ضعف تفکر سیستمیک، فرهنگ فردگرایی، حرفه‌گرایی، پزشک سالاری و الگوهای نقش را در برداشت و موانع عملکردی – ساختاری، زیر طبقات ساختار مدیریت، رشدنیافتگی مدیریت آموزشی، شناخت ناکافی وشواهدضعیف را شامل ‌شد. نتیجه‌گیری: موانع موجود و تأثیر‌پذیری آن‌ها از یکدیگر، با ایجاد یک چرخه‌ی معیوب، مانع اجرای مراقبت تیمی و به‌کارگیری رویکردهای آموزشی تیم محور در نظام سلامت شده‌اند. با توجه به این که تغییرات فرهنگی و نگرشی در طولانی مدت رخ می‌دهند، بایستی با تغییر ساختارها در مدیریت آموزشی و اجرای برنامه‌های آزمایشی آموزش بین‌حرفه‌ای و گسترش تحقیقات وشواهد در این زمینه به سمت تغییر فرهنگ و نگرش و اجرای مراقبت تیمی و رویکردهای آموزشی تیم محور حرکت کرد. Introduction: Today, interprofessional collaboration and team care as well as interprofessional education are inevitable necessities for addressing healthcare needs of societies. Due to a contradiction between the need to provide team care at health centers and the lack of prioritization and implementation of team-based and interprofessional approaches to education in the health system, this study was conducted to explain the barriers to implementation of team care and interprofessional education in Iran from the viewpoints of educational managers of the health system. Methods: This qualitative content analysis study with a conventional approach was carried out from August 2013 to Jul 2014. The data were collected through semi–structured individual interviews with 16 educational managers and researchers in universities of medical sciences and medical education development centers who were selected by purposive sampling. All interviews were transcribed word by word and then analyzed using MAXQD software. Results:Two categories and eight subcategories were identified explaining the barriers to implementation of team care and interprofessional education from educational managers’ viewpoints. The two main categories included cultural-attitudinal barriers and structural-functional barriers. The first category consisted of subcategories such as weakness of systemic thinking, individualistic culture, profession-orientedness, physicians’ predominance, and role models. The second category included subcategories of management structure, stunted growth of educational management, inadequate understanding and weak evidence. Conclusion:Existent barriers and their influence on each other have prevented the implementation of team care and interprofessional education by creating a vicious cycle.
