fa تبیین تجارب پرستاران در زمینه موانع و تسهیل‌کننده‌های آموزش به بیمار آموزش باليني Clinical Teaching پژوهشی اصیل Original research article مقدمه: آموزش به بیمار و به کارگیری مشارکت بیماران در روند درمان در طول بستری به ویژه در زمان ترخیص کاملاً ضروری است. مطالعه حاضر با هدف تبیین تجارب پرستاران در زمینه موانع و تسهیل‌کننده‌های آموزش به بیمار در سال 1388 در بیمارستان شریعتی تهران انجام شد. روش‌ها: مطالعه حاضر یک مطالعه تلفیقی با استفاده از رویکرد دلفی است که در 3 مرحله انجام گردید. نمونه‌ها 25 نفر (شامل 1 مترون، 8 سوپروایزر، 8 سرپرستار و 8 پرستار) بودند. محیط مطالعه بیمارستان شریعتی تهران بوده، روش جمع‌آوری داده‌ها در مرحله اول مصاحبه بدون ساختار، مرحله دوم توسط پرسشنامه حاصل از مرحله اول و در مرحله سوم گروه‌های متمرکز بود. نتایج: میانگین و انحراف معیار سن و سابقه کار واحدهای مورد پژوهش به ترتیب 14/6±20/42 و 20/6±84/14 بود. بر اساس مرحله سوم، مهمترین موانع آموزش بیمار شامل کم بودن دانش و اطلاعات علمی پرستار، کمبود نیروی پرسنلی پرستاری، مشغله زیاد کاری پرستار، ایجاد سیستم نظارت و بازخورد، آموزشی بودن بیمارستان و تحمیل کارهای اضافی به پرستاران، بود. مهم‌ترین تسهیل‌کننده‌ها نیازسنجی از بیمار در هر بخش و ایجاد پروتکل بر اساس آن، ایجاد سیستم نظارت و بازخورد، ارتباطات خوب بین پرستار و بیمار و پزشک، گذاشتن یک پرستار مخصوص آموزش به بیمار به صورت در گردش، برگزاری کلاس‌های آموزشی به عنوان جز ساعت کاری ذکر شد. نتیجه‌گیری: با توجه به چند بعدی بودن موانع و تسهیل‌کننده‌های آموزش به بیمار بر اساس نتایج این مطالعه، انجام اقدامات نظام مند و همه جانبه آموزش به بیمار انجام برای فراهم نمودن امکانات و شرایط طراحی و اجرای برنامه‌های آموزشی و خودمراقبتی بر اساس نیاز مددجویان در بالین توصیه می‌شود. Introduction: Patient education and his/her partnership during hospitalization and at discharge time is essential. The aim of this study is to describe nurses’ experiences of barriers and facilitators of patient education in year 2008 at Shariati Hospital in Tehran (SHT). Methods: The present study is a triangulation research with three steps Delphi method. Participants included 25 nursing staff (1 matron, 8 supervisors, 8 head nurses and 8 nurses). Data were collected in the nursing office and different wards. Data collection methods were an unstructured interview, followed by a questionnaire from interviews, and in the third step, focused groups. Results: Mean and Standard deviation of age and working experience of participants were 42/20±6/14 and 14/84±6/20 years, respectively. Most important barriers according to the third step were: nurses’ low scientific knowledge and information, shortage of nursing manpower, excessive workload, manner of clinical supervision and feedback, and extra tasks demanded due to working in a teaching hospital. The most important facilitators were found to be: developing patient education protocols for each ward, according to needs assessment, revision in supervision and feedback system, establishing a good relationship between patients and nurses and physicians, assigning a rotating nurse for patient education, and running educational courses for nurses as a part of their duty time. Conclusion: The described barriers and facilitators for patient education have a multidimensional nature.
