fa عوامل مؤثر بر عدم انطباق دانش نظری با مراقبت‌های بالینی از دیدگاه پرستاران شاغل در بیمارستان‌های کاشان مديريت آموزشي Educational Management پژوهشی اصیل Original research article مقدمه: فاصله دانش‌آموخته شده و مراقبت‌های بالینی پرستاری یکی از مشکلات پرستاری است. پژوهش حاضر با هدف بررسی عوامل مؤثر بر عدم انطباق دانش نظری با مراقبت‌های بالینی از دیدگاه پرستاران شاغل در بیمارستان‌های شهر کاشان انجام شد. روش‌ها: این مطالعه مقطعی بر روی نمونه تصادفی 384 نفری از پرستاران شاغل در بیمارستان‌های دانشگاه علوم پزشکی کاشان انجام شد. داده‌ها توسط پرسشنامه جمع‌آوری و توسط آمار توصیفی، آزمون تی مستقل و ضریب همبستگی پیرسون تجزیه و تحلیل شد. نتایج: میانگین نمره مربوط به فاصله دانش نظری و مراقبت‌های بالینی پرستاری 62/16±97/59 ( بر مبنای 100) بود که در محدوده فاصله زیاد قرار داشت. از نظر شرکت‌کنندگان، فاصله دانش نظری و خدمات بالینی پرستاران با کسب میانگین 25/18±85/66 در حد زیادی بر سلامت بیماران تأثیر می‌گذارد. بین نمرات مربوط به فاصله دانش‌آموخته شده با خدمات بالینی و نمرات مربوط به تأثیر این فاصله بر سلامت بیمار همبستگی معنادار مشاهده شد (001/0=p و 78/0=r). در همه بخش‌ها «زیاد بودن امور نوشتاری» بیشترین ممانعت را در کاربرد آموخته‌ها ایجاد می‌کرد. در بخش‌های ویژه «عدم حمایت سرپرستار»، در بخش‌های داخلی «کمبود پرستار»، در بخش‌های اورژانس «نامناسب بودن محیط بخش» و در بخش‌های جراحی «به روز نبودن دانش پرستاران» از مهم‌ترین موانع بود. نتیجه‌گیری: تحقیق حاضر نشان داد که از دیدگاه پرستاران، مراقبت‌های بالینی پرستاری از دانش آنها فاصله دارد و این ناهماهنگی بر سلامت بیماران اثر سوء دارد. بنابراین ضرورت دارد تا با اتخاذ راه‌کارهای مناسب تلاش جدی در جهت حذف یا کاهش فاصله بین دانش نظری و عملکرد بالینی پرستاران صورت گیرد. Introduction: The gap between nurses' knowledge and their clinical practice is one of the problems in nursing. The present study was conducted to evaluate the nurses' viewpoints on the barriers to knowledge application in clinical settings. Methods: This cross-sectional study was done on a randomly selected sample of 384 nurses working in hospitals affiliated with Kashan University of Medical Sciences. Data was collected through a researcher made questionnaire and then analyzed using descriptive statistics and independent t-tests and Pearson correlation coefficient. Results: The average score of the gap was 59.97±16.62, (out of 100), that represented a wide gap between the nurses’ knowledge ant their clinical services. According to the participants such gap would severely ( mean score of 66.85±18.25) affect the clients' health. A significant correlation was observed between the gap score and its perceived effect on the clients health (r=0.78, p=0.001). The highest score as a barrier to knowledge application in all wards was given to “too much writing task. However, a non-supportive head nurse, staff shortage, inappropriate environment, and not being up to date were among the important problems in critical care, internal medicine, emergency, and surgical wards respectively. Conclusion: Our nurses believed that there is a gap between the nurses' theoretical knowledge and their clinical services and this gap can affect the patients’ health.
