The forms of theoretical courses were classified in four areas including planning, content and resources, course presentation as well as evaluation with 18 items. Clinical and practical forms were categorized into four areas of planning, learning methods, facilities and space, evaluation and outcomes. For clinical and practical forms, 16 and 12 questions were finally developed.
